Report #42221

[synthesis] Agent quality degrades silently when fallback tools are used instead of primary tools

Track and alert on the ratio of primary vs fallback tool invocations; treat a spike in fallback usage as a high-severity quality incident even if error rates are zero.

Journey Context:
When a primary API \(e.g., a complex code search tool\) degrades in latency, agents often timeout and route to a fallback \(e.g., simple grep\). The agent still completes the task, so success metrics look fine, but the output quality drops significantly because the fallback lacks necessary context. Teams only realize weeks later when reviewing bad code generations.
