[synthesis] Agent silently drops edge-case constraints near context limit

Instrument constraint satisfaction separately from task completion; inject a constraint checklist step after the summarization or compaction step.

Journey Context:
Teams monitor task completion rates, which stay high. But as context windows fill and summarization occurs, the LLM naturally drops low-frequency or late-added constraints \(e.g., specific security rules\) because they have lower attention weight post-compaction. Standard traces show the task finished, missing the constraint violation.
