Report #4208

[agent\_craft] Agent performed an irreversible, expensive, or risky action without checking

Decide autonomously on reversible, low-stakes implementation details; ask for explicit approval on destructive, security-sensitive, or cost-bearing actions.

Journey Context:
Autonomy is a tool, not a virtue. The right boundary is reversibility and blast radius. Read-only inspection and local refactoring are safe to auto-approve; deleting data, changing auth, spending money, or mutating production are not. Permission models that auto-approve reads while hard-stopping mutations reduce friction without removing accountability.
