[frontier] Agent crashes when MCP server updates and removes a tool or changes parameters

Implement capability negotiation at connection time to agree on protocol version and available tools, failing gracefully on mismatch

Journey Context:
Hardcoding tool assumptions breaks when servers evolve. Following MCP lifecycle to exchange capabilities \(protocol version, supported features\) during initialization prevents runtime errors by establishing a contract before any tool invocation.
