[frontier] Agent receives stale file content because tools only read on-demand without subscription

Expose files as MCP Resources with URI schemes and subscription updates instead of ad-hoc tool calls

Journey Context:
Teams start with MCP tools to read files, but hit latency and staleness issues when files change between calls. The fix is using Resources which support subscriptions and URIs like file:// or git://, decoupling data availability from imperative tool invocation and enabling push updates.
