[gotcha] Suggesting ibuprofen or aspirin for fever/pain without screening for asthma
Mandate a contraindication check for asthma before recommending any NSAID \(ibuprofen, naproxen, aspirin\), and default to acetaminophen if asthma status is unknown.

Ibuprofen is a standard OTC go-to for fever, but NSAIDs inhibit cyclooxygenase, shifting arachidonic acid metabolism toward leukotrienes, which can trigger severe, refractory bronchospasm in NSAID-exacerbated respiratory disease \(NERD\). Agents silently recommend it, causing asthma attacks in a subset of the population.
