[gotcha] Relying solely on the FAST mnemonic to rule out stroke

Expand stroke red flags to BE-FAST \(Balance, Eyes\) to capture posterior circulation strokes, and mandate emergency referral for any acute neurological deficit.

Journey Context:
FAST misses ~20% of strokes \(posterior/cerebellar\) which present with vertigo, imbalance, and vision loss but no facial droop or arm weakness. Agents trained only on FAST will give false reassurance to users experiencing severe strokes, delaying time-critical interventions.
