[agent\_craft] Agent follows an outdated plan because the plan in context wasn't updated after a failed step

Force the agent to rewrite its 'plan' or 'scratchpad' context block after every tool execution, explicitly striking through completed steps and updating the next step based on the observation.

Journey Context:
Agents often write a plan at the start and never update it. When a step fails, they still try the next step in the original plan, leading to cascading errors. A mutable, enforced scratchpad ensures the agent's reasoning is always synchronized with reality, preventing rigid adherence to a broken plan.
