Enter plan mode for multi-file or architectural changes; document the approach, file list, and alternatives before editing.

Journey Context:
Agents that edit first often create inconsistent interfaces, orphaned code, and wrong abstractions. A short plan surfaces tradeoffs and lets the user reject a bad direction cheaply. The overhead is only justified for non-trivial changes; for one-line fixes, planning is waste. The plan file becomes the contract for the implementation.
