[gotcha] Suggesting ibuprofen or other NSAIDs as safe pain/fever alternatives for asthmatics
Before recommending NSAIDs, explicitly warn about NSAID-exacerbated respiratory disease \(NERD\). Up to 20% of asthmatics may experience severe bronchospasm with NSAIDs. Acetaminophen is generally safer.

Agents often rank NSAIDs as standard alternatives to acetaminophen. For a subset of asthmatics, especially those with nasal polyps \(Samter's triad\), NSAIDs trigger catastrophic asthma attacks. The counter-intuitive part is that a simple painkiller can cause respiratory failure in this specific demographic, and users rarely volunteer their asthma history unprompted when asking for a backache remedy.
