[architecture] Orchestrator routing tasks to the wrong specialized agent due to ambiguous user intent

Have the orchestrator generate a structured routing plan with a confidence score; if confidence is below a threshold, route to a clarifier agent instead of guessing.

Journey Context:
A single LLM router often hallucinates a destination when intent is vague. Rather than forcing a choice and triggering a cascade of errors in the wrong sub-agent, the router must be allowed to say 'I don't know.' Routing to a clarifier agent breaks the ambiguity early, adding an extra step but drastically reducing downstream failure rates.
