[agent\_craft] Writing PR descriptions that only restate the code changes without explaining the motivation

Structure PR descriptions with 'Why' \(motivation/context\) before 'What' \(changes\). Link to the relevant issue tracker.

Journey Context:
The diff already shows \*what\* changed. The human reviewer needs to know \*why\* the change was made to evaluate if the approach is correct. Omitting the 'why' forces the reviewer to reverse-engineer the intent from the code, which is slow and error-prone.
