[research] Context drift and hallucination during multi-hop tool workflows

Enforce 'Chain-of-Note' where the agent explicitly writes down the observed evidence from Tool A before calling Tool B, and strictly derives the final answer only from these written notes.

Journey Context:
In multi-hop retrieval, agents often forget or conflate intermediate retrieved documents, defaulting to parametric memory. Forcing the model to assess the relevance of each retrieved document and write a reading note before synthesizing the final answer drastically reduces ungrounded hallucinations by creating a verified, isolated intermediate state.
