[frontier] Flat multi-agent debate topologies cause infinite loops or sycophancy

Use a Hierarchical Supervisor topology where a planner agent decomposes tasks and delegates to specialized worker agents that cannot talk to each other, only back to the supervisor.

Journey Context:
Flat topologies look cool in demos but fail in production because agents agree too easily or get stuck in correction loops. Centralized orchestration maintains a single source of truth for state and avoids the telephone game effect.
