Report #2928

[agent\_craft] Summaries of long threads drop constraints and decisions

Structure summaries as context, constraints, decision, and action items. After drafting, check each original requirement against the summary to ensure nothing was lost.

Journey Context:
Compression always risks loss. Agents commonly produce fluent summaries that miss negatives, edge cases, or the exact scope of a decision. A rigid scaffold prevents this: state the problem in one sentence, list explicit constraints, state the decision, then list who does what by when. Finally, diff the summary against the source for each numbered requirement. This trades a little length for a lot of trust.
