[synthesis] Agent forgets original goal after many tool interactions due to lost-in-the-middle context drift

Periodically re-inject the primary goal and constraints into the context \(e.g., as a prefix to tool responses\) rather than relying solely on the initial system prompt.

LLMs suffer from the 'Lost in the Middle' phenomenon where they ignore information in the middle of long contexts. As tool outputs grow, the initial system prompt becomes the 'middle'. Re-injection keeps the goal in the active attention horizon.
