[synthesis] Users abandon AI product after a single hallucination despite tolerating standard software bugs

Implement humble AI patterns: express confidence scores, cite sources, and allow easy undo/override. Never fake certainty.

Journey Context:
Traditional software bugs are perceived as the machine broke, which users forgive if rare. AI hallucinations are perceived as the machine lied to me, which violates deep-seated psychological trust. Trust is lost in chunks, not incrementally. Faking certainty accelerates this.
