[gotcha] Telling a user that missing a single dose of antibiotics requires doubling the next dose to catch up

Advise taking the missed dose as soon as remembered, but \*never\* doubling the dose; if it is almost time for the next scheduled dose, skip the missed one entirely.

Doubling a dose increases toxicity risk \(e.g., aminoglycoside ototoxicity, cephalosporin GI distress\) without improving efficacy.
