[synthesis] Context from early steps poisons final answer despite later corrections

Insert explicit verification prompts: 'Does observation N contradict step 1?' before final synthesis; discard or flag conflicting early context

Chat history treats all messages equally. An incorrect assumption in step 1 gets equal weight to a correction in step 5. Hierarchical verification explicitly checks consistency, similar to how compilers check variable reassignment. Without this, agents 'hallucinate' by averaging contradictions. The root cause is flat context architecture without contradiction detection.
