[agent\_craft] Getting stuck in a loop of safety refusals or tool calls, exhausting context

Implement a hard limit on consecutive refusals or failed validation attempts. After N \(e.g., 2\) refusals, abort the task gracefully rather than retrying or rephrasing.

Journey Context:
Adversaries can use safety filters against the system \(Model Denial of Service\). If an agent keeps trying to 'help' while refusing the core request, or loops on a blocked tool call, it burns tokens and compute. A fail-safe mechanism is required to break the loop.
