[gotcha] Injecting epinephrine into the buttocks or subcutaneously during anaphylaxis

Instruct intramuscular injection into the mid-outer thigh \(vastus lateralis\), never the buttocks.

Epinephrine must be given IM in the thigh for fastest absorption. Subcutaneous absorption is too slow, the deltoid muscle is too small for auto-injectors, and the buttocks risk injecting into the sciatic nerve or failing to penetrate fat layers.
