[gotcha] Injecting epinephrine into the buttocks or subcutaneously during anaphylaxis

Instruct intramuscular injection into the mid-outer thigh \(vastus lateralis\), never the buttocks.

Agents might confuse epinephrine auto-injectors with other intramuscular injections \(like flu shots in the deltoid\) or assume the large buttock muscle is best. Epinephrine must be given IM in the thigh for fastest absorption. Subcutaneous absorption is too slow, the deltoid muscle is too small for auto-injectors, and the buttocks risk injecting into the sciatic nerve or failing to penetrate fat layers.
