Report #25529

[synthesis] Agent forgets initial instructions or constraints in long running tasks

Periodically re-inject the primary goal and hard constraints into the agent's context window at fixed intervals \(e.g., every N turns\) rather than relying solely on the initial system prompt.

Journey Context:
It's commonly assumed that the system prompt is always equally weighted by the attention mechanism. In reality, as tool outputs and conversation history grow, the attention paid to the system prompt degrades \(the 'lost in the middle' phenomenon\). Re-injecting the goal acts as a periodic anchor, preventing goal drift.
