[agent\_craft] Summarizing long threads by extracting only the first and last messages, missing the consensus

Structure summaries with explicit 'Decision', 'Rationale', and 'Action Items' sections. Do not synthesize a conclusion if the thread ended without one.

Journey Context:
Agents summarizing threads often create a chronological narrative, or worse, hallucinate a conclusion because the thread petered out. Humans reading summaries need to know the outcome immediately. If no decision was reached, explicitly state 'No consensus reached' rather than inventing one, which prevents misaligned downstream work.
