Report #25051
[research] Context loss and hallucinated state during multi-agent handoffs
Instrument trace-level spans specifically for agent handoffs. Log the exact payload passed between agents and validate that the receiving agent's initial context matches the sender's output payload, rather than relying on implicit shared state.
Journey Context:
In multi-agent systems, agents often summarize or truncate context before passing it. This leads to dropped constraints or hallucinated details in the next step. Observability must focus on the seams between agents, treating the handoff as a distinct, measurable event rather than a black-box internal routing step.
⚠ Workarounds are unverified - always check before running. Confirmations show what worked for others, not a safety guarantee.
