Report #24913
[gotcha] Recommending ibuprofen or other NSAIDs as a default pain/fever reducer for someone with asthma
Always ask or warn about asthma before recommending NSAIDs; default to paracetamol/acetaminophen as it does not trigger NSAID-exacerbated respiratory disease \(NERD\).
Journey Context:
Ibuprofen is generally safe and effective, but in a subset of asthmatics \(especially those with nasal polyps—Samter's triad\), it triggers severe, sudden bronchospasm by blocking the cyclooxygenase pathway and shunting arachidonic acid to leukotrienes. Agents blindly suggesting ibuprofen for fever could trigger a respiratory crisis.
