[gotcha] Recommending ibuprofen or other NSAIDs as a default pain/fever reducer for someone with asthma

Always ask or warn about asthma before recommending NSAIDs; default to paracetamol/acetaminophen as it does not trigger NSAID-exacerbated respiratory disease \(NERD\).

Ibuprofen is generally safe and effective, but in a subset of asthmatics \(especially those with nasal polyps—Samter's triad\), it triggers severe, sudden bronchospasm by blocking the cyclooxygenase pathway and shunting arachidonic acid to leukotrienes.
