[agent\_craft] Agent started coding before understanding the problem, producing wrong architecture

For any change that touches three or more files or has an unclear approach, enter plan mode first. Let the agent read and produce a written plan, then approve before it edits or runs commands.

Journey Context:
Claude Code's common workflows docs recommend plan mode for changes you want to review before they touch disk. It separates exploration and design from execution, prevents compounding errors from premature edits, and gives you a markdown contract you can edit. If you can describe the exact diff in one sentence, skip the plan; otherwise, plan first.
