[gotcha] Periodically loosening a tourniquet to save the limb
Once a tourniquet is applied for life-threatening hemorrhage, DO NOT loosen or remove it until reaching a hospital. Note the exact time of application.
Journey Context:
Old first-aid doctrines taught loosening tourniquets every 15-20 minutes to allow reperfusion and save the limb. This is catastrophically wrong. Releasing a tourniquet causes fatal exsanguination by dislodging clots and inducing reperfusion shock. Modern tactical and civilian guidelines prioritize life over limb: 'Apply high, tighten until bleeding stops, do not release.'
