[synthesis] Agent confidently persists in wrong approach for multiple consecutive steps

Inject a devil's advocate system prompt or a secondary verification step after 2 consecutive failed attempts. Force the agent to explicitly write down why its previous approach failed before generating the next action.

Journey Context:
LLMs exhibit sycophancy and confirmation bias; if they commit to a path, they rationalize failures as minor tweaks needed rather than fundamental flaws. Simply asking are you sure? does not work. Forcing a structured post-mortem of the failure breaks the chain of confident wrongness by resetting the logical priors.
