[research] Conflating distinct entities that share similar names or contexts

Require the model to extract and explicitly define the exact entity \(with unique identifiers if possible\) from the context before answering questions about it.

Journey Context:
LLMs struggle with entity disambiguation, often blending facts about Entity A and Entity B if they share a domain. Forcing explicit entity resolution as a prerequisite step reduces bleed-over of factual attributes.
