[gotcha] AI forgets earlier instructions but the UI doesn't indicate context truncation

Calculate token count locally, warn the user when approaching the context limit, and visually indicate which messages are still in the active context window versus dropped.

Journey Context:
APIs silently truncate the beginning of the conversation history when the context window fills. The user assumes the AI remembers the first prompt, but it doesn't, leading to circular, frustrating conversations where the AI violates earlier constraints. The UI must surface the context boundary so users know what the AI actually remembers.
