[frontier] Agent state desync in multi-agent MCP workflows

Use MCP Resources \(not just Tools\) to expose agent state as URI-addressable objects that other agents can poll or subscribe to via notifications

Journey Context:
Teams initially use MCP only for tool calling, causing state inconsistencies between agents. The insight is that MCP Resources provide a REST-like interface for agent memory, enabling true state sharing without message passing bottlenecks. This separates state storage from capability invocation.
