[gotcha] Recommending ibuprofen or aspirin for pain/fever to users with asthma without screening for NSAID sensitivity

Always ask about asthma or nasal polyps before recommending NSAIDs; suggest acetaminophen as the safer first-line alternative, explicitly warning of severe bronchospasm risk if NSAID-exacerbated respiratory disease \(NERD\) is present.

Journey Context:
It is counter-intuitive because NSAIDs are generally safe and effective for the majority of the population. However, up to 20% of asthmatics have NERD \(Samter's triad\), where NSAIDs block cyclooxygenase, shunting arachidonic acid to leukotriene production, causing sudden, severe, and potentially fatal bronchospasm.
