[agent\_craft] Vague instructions like 'be careful' do not improve output quality

Replace fuzzy meta-instructions with concrete constraints: JSON schemas, enumerated choices, token budgets, and post-hoc validation checks.

Journey Context:
Telling a model to 'be thorough' or 'think step by step' is weak signal. What works is specifying the shape of the answer and validating it. Structured outputs, strict schemas, and programmatic checks turn a wish into a contract. The OpenAI structured-output feature exists precisely because models follow form more reliably than they follow tone.
