[synthesis] Agent fixates on local test errors and loops on symptom fixes instead of root causes

Limit consecutive error-fix cycles \(e.g., max 3\). On the Nth failure, force the agent to stop running tests and instead output a high-level 'Root Cause Analysis' of the entire module before touching code again.

Journey Context:
The 'fix the error' loop is addictive for agents because it provides clear, immediate feedback. But it leads to whack-a-mole. Breaking the loop forces the agent to switch from System 1 \(reactive\) to System 2 \(analytical\) thinking.
