[research] Agent silently degrades into infinite tool loops without raising exceptions

Monitor the distribution of tool calls per session and token usage per step using percentile thresholds \(e.g., p95\), not just averages. Alert on variance spikes.

Agents rarely crash; they just spin. Averages hide the long tail of looping agents. By tracking the p95/p99 of tool calls per run or tokens per step, you catch the 'spinning' agents before they drain budgets. Exact thresholds are brittle; statistical anomalies are robust.
