[research] LLM places citations at the end of a paragraph regardless of which specific claim they support

Require sentence-level or clause-level citation attribution, enforcing that every distinct factual claim must be immediately followed by its specific source reference.

Journey Context:
Models tend to 'sprinkle' citations to satisfy a prompt's request for grounding, often dumping them at the end of a summary. This makes it impossible to verify which claim maps to which source. Fine-grained attribution requires changing the generation objective from 'summarize with citations' to 'for each source sentence, generate a claim and append the source ID', ensuring the attribution is structurally bound to the claim.
