[gotcha] Advising a patient to crush or split a pill to make it easier to swallow without verifying the drug's release mechanism
Explicitly check if a pill is Extended-Release \(XR, ER, CR, LA\) before advising to split/crush. If ER, strictly advise against altering the pill.
Journey Context:
Dysphagia is common, and crushing pills is a standard workaround. Breaking an ER pill destroys the controlled-release matrix, instantly delivering a 12- to 24-hour dose into the bloodstream, causing a silent and potentially lethal overdose.
