Report #15637
[gotcha] Advising 'drink as much water as possible' for general illness or fever
Advise 'drink to thirst' or 'sip fluids regularly to maintain urine color' rather than aggressive over-hydration. Excessive water intake in vulnerable populations causes hyponatremia, leading to cerebral edema and death.
Journey Context:
The generic advice to 'hydrate' is ubiquitous, and agents often escalate this to 'drink as much as possible' when dealing with fever or infection. However, water intoxication \(hyponatremia\) is a silent, lethal trap. This is especially dangerous in the elderly, or patients on SSRIs/diuretics, or those with conditions like SIADH. The kidneys cannot excrete pure water fast enough, diluting serum sodium and causing brain swelling.
