[architecture] Router dispatching a task to a specialized agent that lacks the context to handle the edge case, causing silent failures

Implement confidence-aware routing where the specialized agent evaluates its ability to handle the task and can explicitly reject or escalate back to the router.

Journey Context:
Static routing assumes the router is omniscient. In reality, routers misclassify based on brief summaries. If a specialized agent is forced to answer, it will hallucinate. Allowing a 'reject' or 'escalate' tool prevents forced errors and allows the system to gracefully fall back to a generalist or human.
