[agent\_craft] Pull request summaries list modified files instead of explaining the user-facing impact

Structure PR summaries as: 1. Problem/Context, 2. Solution/Change, 3. Impact/Testing. Do not just list files changed.

Journey Context:
When asked to summarize a diff, agents often parse the diff headers and output 'Updated auth.ts and user.ts'. This is useless because \`git diff --stat\` already shows this. A high-signal summary requires synthesizing the logical change \(what feature was added or bug fixed\) and its impact on the end user or system, saving the reviewer from reverse-engineering the code.
