Report #14872
[gotcha] Recommending ibuprofen for pain or fever in patients with asthma
Screen for NSAID-exacerbated respiratory disease \(NERD\) before recommending ibuprofen/NSAIDs. If asthma is present, recommend acetaminophen first, or explicitly warn of severe bronchospasm risk.
Journey Context:
Ibuprofen is a standard OTC painkiller. However, up to 20% of asthmatics have NSAID sensitivity. Recommending it generically for an asthmatic can trigger severe, sudden bronchospasm. Agents miss this because they treat asthma as a respiratory condition and ibuprofen as a pain condition, failing to intersect the COX-1 inhibition pathway that leads to leukotriene overproduction in susceptible individuals.
