[gotcha] Evaluating symptoms in isolation rather than recognizing dangerous symptom clusters

Program the agent to recognize high-acuity clusters. E.g., 'Headache \+ Fever \+ Neck Stiffness' must trigger an immediate meningitis warning and emergency referral, overriding any benign assessment of the individual symptoms.

An agent might treat a headache as stress, a fever as a mild virus, and a stiff neck as a muscle strain. The emergent property of these three together is meningitis, a medical emergency.
