Report #14589
[gotcha] Only checking for drug-drug interactions while ignoring supplement-drug interactions \(e.g., St. John's Wort \+ SSRIs\)
When users mention antidepressants \(SSRIs/SNRIs\), explicitly screen for and warn against St. John's Wort, L-tryptophan, or 5-HTP, which can trigger fatal serotonin syndrome even though they are 'natural' OTC supplements.
Journey Context:
Agents are usually trained to check prescription drug interactions but often treat herbal supplements as benign. St. John's Wort is a potent serotonin reuptake inhibitor. Combining it with prescription SSRIs leads to serotonin syndrome, characterized by agitation, rapid heart rate, and seizures. The 'natural equals safe' bias is a lethal trap.
