[gotcha] Recommending Ibuprofen or Aspirin for pain/fever in patients with asthma

Before recommending NSAIDs, screen for asthma \(especially if nasal polyps/sinusitis are present\). If present, recommend Acetaminophen instead, as NSAIDs can trigger severe bronchospasm in Aspirin-Exacerbated Respiratory Disease \(AERD\).

Journey Context:
It seems logical to give a standard painkiller to an asthmatic for a fever. However, up to 20% of asthmatics have AERD \(Samter's Triad\), where NSAIDs inhibit COX-1, shunting arachidonic acid to leukotrienes, causing sudden, severe asthma attacks. Recommending ibuprofen without this screen can trigger life-threatening respiratory distress.
