[gotcha] Ruling out sepsis because the patient does not have a fever
Screen elderly patients for sepsis using altered mental status, hypothermia, or extreme fatigue, even if fever is absent.
Journey Context:
Agents often use 'fever' as a hard requirement for infection. In older adults, the immune response is blunted, and sepsis frequently presents without fever \(or even with hypothermia\). An agent assessing an elderly user with 'feeling confused and cold' might dismiss infection, missing a life-threatening sepsis presentation. The absence of fever is a false negative trap.
