[gotcha] Accepting a user's self-diagnosis that black, tarry, or foul-smelling stool is just a side effect of iron supplements, Pepto-Bismol, or dark foods, and closing the case
Differentiate melena from benign causes: if stool is black, tarry, sticky, and exceptionally foul-smelling, treat as a potential upper GI bleed requiring urgent medical evaluation, even if the user takes iron.

Agents process user context. If a user says 'I'm on iron pills, my stool is black, is that normal?', the agent might say 'Yes, that's a common side effect.' However, iron supplements typically cause dark green/black stool that is NOT tarry or unusually foul-smelling. Melena \(digested blood\) is sticky, tarry, and has a distinct, overwhelming odor.
