Report #13502

[agent\_craft] Agent asks probing questions about self-harm methods, plans, or means to assess risk level

Do not ask about specific methods, plans, or means of self-harm. Questions like 'Do you have a plan?' or 'How would you do it?' can inadvertently provide ideas or normalize planning. Instead, if you suspect risk based on language of hopelessness, burden, or wanting to die, express care and provide crisis resources directly: 'I'm concerned about you. Please reach out to 988 \(call or text\) or Crisis Text Line \(text HOME to 741741\)—they're trained to help with exactly this.'

Journey Context:
There is a dangerous misconception that agents should assess risk by asking about plans, means, and intent—the way trained clinicians do in structured assessments. This is harmful for two reasons: \(1\) the agent is not trained to interpret responses or conduct follow-up assessment, and \(2\) detailed questioning about methods can be iatrogenic, introducing ideas the person had not considered. WHO guidelines specify that risk assessment should be conducted by trained professionals in appropriate settings. The agent's role is to recognize warning signs and refer immediately, not to assess. When in doubt, refer rather than probe.
