[gotcha] Silent tool execution failures or infinite loops without agent visibility

Implement strict timeouts and mandatory structured logging for all MCP tool calls. The agent must have a fallback mechanism if a tool hangs or returns empty/error without crashing the entire agent loop.

Journey Context:
MCP servers can hang or return ambiguous errors. Agents often get stuck in loops, repeatedly calling a failing tool with the same arguments because they don't understand the tool is broken, leading to token exhaustion. Telemetry on tool latency and error rates is critical to break these loops.
