Report #12407

[agent\_craft] Agent ignores instructions placed in the middle of long system prompts

Place critical directives \(e.g., 'Always use type hints'\) at the very beginning or end of the context window; bury metadata \(file lists, git history\) in the middle.

Journey Context:
Models exhibit U-shaped attention over long contexts. Placing instructions in the middle effectively hides them from the model's attention mechanism. Many agents randomly shuffle context items, destroying task adherence. This pattern is often mistaken for 'model stubbornness' when it's actually positional blindness.
