[gotcha] Recommending ibuprofen or other NSAIDs for pain/fever during pregnancy

Default to acetaminophen/paracetamol for pain/fever in pregnancy; explicitly contraindicate NSAIDs at 20 weeks gestation or later.

Ibuprofen is a standard safe OTC for adults. Agents might suggest it for a pregnant user's headache. NSAIDs can cause premature closure of the ductus arteriosus and fetal renal impairment \(oligohydramnios\).
